To request an appointment, please select a preferred location or call 443-787-4228.
New patients should arrive 30 minutes prior to scheduled appointment time and returning patients should arrive 15 minutes prior to schedule appointment time. There are several forms and insurance documents which need to be completed at the first visit. Routine forms also need to be updated at least annually, sometimes more often.

Cancellation/No-Show Policy

We respectfully request that you cancel appointments with 24-hour notice, if possible. We appreciate early notification of your change of plans so that we may offer that appointment to another patient in need. We will be happy to assist you in rescheduling your appointment for another date/time.

Guidelines for Your First Appointment

  • Please arrive 15-30 minutes prior to your scheduled visit for completion of the registration process, medical history paperwork, and payment of co-pay (if applicable).
  • Bring a detailed medication list of your prescription bottles with you. Important information includes the name of the medication, dosage, the number of times it is taken daily, and the pharmacy contact information.
  • Bring any forms you may need completed with you at the time of the visit. This includes handicapped parking forms, daycare/school forms, mail-in pharmacy requests, etc.
  • If you are diabetic or have high blood pressure and you take blood sugar and/or blood pressure readings at home, bring in a written record of those readings to your appointment.
  • Bring any old records you may have, including recent lab results.
  • Bring an immunization record. If you do not have a copy, you may consider asking the school for a copy if the child is of school-age or request one from your previous physician’s office.
  • Please bring your previous physician’s office address and phone number. This information is necessary to request old records from their office.
  • Review your rights and responsibilities as a patient.

CityCare Health Care Providers encourage patients to speak openly with the health care team, take part in their treatment choices, and promote their own safety by being well-informed and involved in their care. Because we want our patients to think of themselves as partners in their care, we want them to know their rights as well as their responsibilities. We invite our patients and their families to join us as active members of their care team.

Patient Rights

  • The right to receive considerate, respectful and compassionate care in a safe setting regardless of age, gender, race, national origin, religion, or disabilities.
  • The right to receive care in a safe environment free from all forms of abuse, neglect, or mistreatment.
  • The right to be called by your proper name and to be in an environment that maintains dignity and adds to a positive self-image.
  • The right to be told the names of your health care providers, nurses, and all other members of the health care team directing and/or providing your care.
  • The right to be told by your health care provider about your diagnosis and possible prognosis, the benefits and risks of treatment, the alternatives for care or treatment, and the expected outcome of treatment, including unexpected outcomes.
  • You have the right to give written informed consent before the start of any non-emergency procedure and/or treatment. You have the right to refuse treatment to the extent permitted by law. You have the right to be informed of the medical consequences of refusing treatment.
  • You, your family and friends with your permission, have the right to participate in decisions about your care, your treatment, and services provided.
  • You have the right to respectful, responsive care directed at fostering your comfort and dignity, including assessment of pain, and responding to you and your family
